Tv Ratings? - dvr count for ratings

I wanted to know how the Nielsen ratings in the can "watch" a show confidence when you DVR'ed. Is it because it is counted, has been found or an account where you are looking for? For things like American Idol, Shure I count as one year, although he has been recording and I saw something else. But then I give you notice 2 of the TV at the same time ?????
